Panchakarma in Sri Lanka
Panchakarma — five actions — is the central purification therapy of Ayurvedic medicine, and the treatment Ayurveda Sarana Beach Hospital was built around. Every programme is prescribed and reviewed daily by Dr. Ravindra Karandeniya, who has practised in Negombo for more than twenty-two years.
This page explains what authentic Panchakarma actually involves, so you can judge for yourself whether what you are being offered elsewhere is the real protocol.
What Panchakarma actually is
Panchakarma is a medical procedure, not a spa package. Its purpose is to remove accumulated metabolic waste — Ama in Sanskrit — from the deep tissues of the body, and then to restore the balance of the three doshas: Vata, Pitta and Kapha. Ayurveda holds that most chronic disease begins when Ama accumulates faster than the body can clear it, and that no amount of medication resolves the underlying problem while that residue remains.
The word describes five specific cleansing actions. What surrounds them matters equally: several days of preparation before, and a carefully staged recovery afterwards. A programme that offers the five procedures without those two phases is not Panchakarma, whatever it is called in the brochure.
This is also why length cannot be negotiated away. The classical texts, and Dr. Ravindra's own clinical experience, put the minimum at fourteen days. We would rather tell you that honestly than sell you a three-day version that will not work.
The three phases of treatment
Panchakarma is sequential. Each phase depends on the one before it.
Poorva Karma
Preparation3–7 daysNothing is expelled from the body until it has first been loosened and drawn back toward the digestive tract. This preparatory stage combines Snehana (internal and external oleation with medicated ghee and oils) and Swedana (herbal steam therapy) to soften hardened toxins and mobilise them. Skipping or shortening this phase is the single most common failing of short 'Panchakarma' packages — the main procedures then have nothing prepared to remove, and the guest experiences depletion rather than relief.
Pradhana Karma
The main procedures5–14 daysThe five actions themselves. You will not receive all five — that is a common misunderstanding. Dr. Ravindra prescribes only those indicated by your constitution, your presenting condition, your age and your strength. A guest with inflammatory joint pain and one with chronic sinusitis leave with genuinely different programmes.
Paschat Karma
Restoration3–7 daysThe phase most often cut short, and the one that determines whether results last. Digestive fire (Agni) is rebuilt gradually through a staged diet beginning with thin gruels, alongside rejuvenating herbs (Rasayana) and a measured return to normal activity. Guests who leave immediately after the main procedures typically lose much of what they gained within weeks.

Vamana
Therapeutic emesisKaphaA controlled, medically induced vomiting that clears accumulated Kapha from the stomach and chest. Preceded by several days of internal oleation and steam so the toxins have already loosened and moved toward the digestive tract. Indicated for chronic congestion, asthma, persistent sinus problems, skin conditions and some metabolic disorders.
Virechana
Therapeutic purgationPittaA medicated purgation that clears excess Pitta from the liver, gall bladder and small intestine. Usually the gentlest of the five to undergo, and one of the most widely applicable. Commonly indicated for inflammatory skin conditions, chronic acidity, liver congestion, migraines and inflammatory joint pain.
Basti
Medicated enemaVataConsidered the single most important of the five procedures, because Vata governs movement in the body and the colon is its principal seat. Medicated oils and herbal decoctions are administered rectally in a carefully sequenced course. Central to the treatment of arthritis, chronic lower back pain, sciatica, constipation, neurological complaints and anxiety.
Nasya
Nasal administrationHead & neckMedicated oils and herbal preparations delivered through the nasal passages, which Ayurveda regards as the direct route to the head. Used for sinusitis, chronic headaches and migraine, cervical spondylosis, insomnia, and clouded thinking or poor concentration.
Raktamokshana
Blood purificationRakta (blood)The most selective of the five, prescribed only where the blood itself carries the disorder. Applied in certain chronic skin diseases, localised inflammation, varicose veins and some forms of gout. Dr. Ravindra prescribes it only where the classical texts genuinely indicate it — not as a routine part of every programme.

Why Negombo for Ayurveda in Sri Lanka
Negombo sits on Sri Lanka's west coast, roughly thirty minutes by road from Colombo Bandaranaike International Airport. For a guest arriving on an overnight flight from Frankfurt, Vienna or London, that difference is not trivial — most Ayurvedic centres in Sri Lanka are three to five hours further south, which means a long transfer at the end of an already long journey.
The coastal climate matters clinically too. Panchakarma relies heavily on warmth — oleation, steam therapy and heated oils — and Negombo's consistent tropical temperatures support the treatments year-round rather than seasonally.
